If you have health insurance coverage, we will ask you to provide us with the name of your insurance company, subscriber(member) number, and group number so we may verify your insurance coverage and inform you of any copay/deductible information as well as any limitations to your policy on your first visit prior to the exam. If you are not the subscriber to the policy, we will need your spouse and/or parent's information also(whichever is applicable). Please allow us time to verify any insurance coverage you have prior to your initial visit. For instance, Blue Cross/Blue Shield policies have different copays, deductible, & limitations(one policy may have a $25 copay whereas another may have a deductible). In addition to health insurance, we accept worker's compensation and auto accident insurance. We will file any claims from our office for you.
